RESUMO

The present article reviews the literature regarding tetracycline staining of the dentition and its esthetic management. A total of 49 articles were identified in the initial search, and 22 articles met the eligibility criteria of this narrative literature review. Tetracycline staining results from its use in children between the ages of 6 months and 6 years, when tooth crowns are formed; the effects are dependent on the dosage, duration of treatment, stage of tooth mineralization, and activity of the mineralization process. Tooth bleaching is always the first treatment option for low-grade cases of tetracycline staining. Restorative procedures are indicated for severe staining. The article also describes the treatment of severe staining affecting the maxillary anterior dentition of a 48-year-old man who reported the systemic administration of tetracycline during childhood. Direct composite resin laminate veneers were proposed to achieve immediate, highly esthetic restorations. A combination of different resinous materials in different shades was applied to meet the needs of the patient. The use of direct composite resin laminate veneers satisfied the functional and esthetic demands of the patient at a lower cost than ceramic materials.

OBJECTIVES: To compare, in vitro, resin cement excess removal techniques at the veneer-tooth interface. MATERIALS AND METHODS: Anterior human teeth were restored with ceramic veneers and randomly divided according to the following techniques (n = 10): removal of excess resin cement with brush and dental floss, followed by light-curing with Valo (Group 1) or Elipar (Group 2) for 1 min and 40 s; tack-curing with Valo (Group 3) or Elipar (Group 4) for 1 s; and tack-curing with Valo (Group 5) or Elipar (Group 6) for 5 s. The tack-curing was followed by removal of excess with probe and dental floss and light-curing for 1 min and 40 s. The area of excess resin cement (mm2) was measured in micro-CT images using AutoCAD program. The failures at the cervical margin in the X, Y, and Z axes (µm) of greater value were measured using the DataViewer program. The specimens were submitted to microleakage with 2% basic fuchsin. RESULTS: According to the Kruskal-Wallis and multiple comparison test, the highest area of excess resin cement was found in Group 1 (5.06 mm2), which did not differ statistically from Groups 2 (3.70 mm2) and 5 (2.19 mm2). Groups 2, 3 (1.73 mm2), 4 (1.14 mm2), and 5 (2.18 mm2) did not differ statistically. Group 6 (0.77 mm2) obtained the lowest value, which did not differ statistically from Groups 3 and 4. According to the Kruskal-Wallis and Dunn test, there was no significant difference in failures in X (p = 0.981), Y (p = 0.860), and Z (p = 0.638) axes and no significant difference in microleakage (p = 0.203) among the groups. CONCLUSIONS: Tack-curing for 1 s or 5 s, followed by removal of excess resin cement using a probe and a dental floss, tended to result in a lower amount of excess material around the margin. CLINICAL RELEVANCE: The technique used for resin cement excess removal influences the amount of excess leaved at the veneer-tooth interface. Tack-curing for 1 s or 5 s is recommended to mitigate the excess resin cement.

The longevity of ceramic laminate veneers can be influenced by several factors, which can result in the need for a removal process. Laser removal has emerged as a good alternative to facilitate the procedure, and its repercussions on tooth enamel have been investigated. We aimed to evaluate the efficacy of erbium lasers for debonding ceramic laminate veneers without damaging the tooth enamel. This systematic review based on the PICOS model adhered to the PRISMA statement. The PubMed/MEDLINE, Web of Science, Embase, and Scopus databases were systematically searched until December 1, 2022, and 2902 studies were retrieved. After screening, four in vitro studies that analyzed the dental morphology using scanning electron microscopy, optical analysis, stereomicroscopy, or x-ray dispersion spectroscopy were included. The risk of bias was assessed using the Cochrane Collaboration tool. Our findings suggest that erbium lasers are useful for ceramic laminate veneer removal without damaging the tooth enamel. However, the removal is influenced by the type and thickness of ceramic and type of cement used. It could be concluded that the application of Erbium laser did not promote superficial changes in the dental enamel. This effect was observed in all analysis performed.

Intrinsic discoloration can be formed due to many reasons, and by clinical evaluation and history from the patient, the exact cause of the discoloration can be known and one of the reasons that can make the intrinsic discoloration is the high level of fluoride intake. Two siblings had come to the Department of Restorative Dentistry clinics; they had the same compliance which is severe dental fluorosis, caries, and malocclusion of their teeth. What is interesting and worrying is that some patients with severe dental fluorosis show higher levels of caries, which can be explained as the enamel becoming porous with severe fluorosis. The possible treatment options were discussed with the patients. According to their financial concern and their desire to get their teeth done as soon as possible, the use of direct composite veneer for the treatment of anterior teeth was decided. Long-term clinical trials are required to assess the efficacy of various treatment options for fluorosis of different severity.

STUDY SELECTION: Partial laminate veneers, defined as small ceramic restorations adhesively luted onto unprepared anterior teeth, are an interesting and conservative alternative to conventional ceramic and composite resin veneers in the anterior region. This literature review aimed to summarize the available laboratory and clinical data on ceramic partial laminate veneers. An electronic search of the MEDLINE/PubMed, EBSCO, and Web of Science databases was conducted. The keywords used were "partial veneer," "partial laminate veneer," "ceramic fragment," and "sectional veneer." The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed. The risk of bias in the included studies was assessed using the QUIN tool. RESULTS: Of the 266 identified articles, only 16 met the inclusion criteria (ten clinical reports, five laboratory studies, and one retrospective clinical study). To date, no randomized controlled clinical trials have been conducted. Most laboratory studies displayed a low risk of bias, with partial laminate veneers rendering adequate strength and color stability. Clinical reports have shown large variability in material selection, luting, and finishing/polishing protocols. CONCLUSIONS: Low-quality evidence is available for ceramic partial laminate veneers. Available data from laboratory studies suggest good mechanical and optical performances comparable to those of conventional ceramic and composite resin veneers. Further clinical studies with longer follow-up periods are warranted.

Determining the appropriate technique for diastema closure is challenging, and the decision must be evidence based. The objective of these case reports is to describe different approaches to diastema closure using direct and indirect techniques, focusing on the characteristics of the patient and clinical requirements to guide treatment. In the first case, a 16-year-old patient had multiple diastemas in the maxillary anterior dentition. The clinical evaluation revealed microdontia of the lateral incisors and malpositioned teeth. The treatment included tooth whitening and placement of composite resin veneers using a direct technique. In the second case, a 54-year-old patient displayed a disharmonious and esthetically compromised smile due to small teeth, color changes, multiple diastemas, incisal wear, and severe dentogingival disproportion. Based on the patient's expectations, the patient's age, and the presence of a "black triangle" interdental space, a multidisciplinary restorative treatment was proposed, including gingivoplasty, tooth whitening, and placement of ceramic laminate veneers using an indirect technique. Both approaches achieved successful esthetic rehabilitation and diastema closure with minimal intervention. The choice of procedure and restorative material, as well as the need for tooth preparation, varied based on the clinical requirements, patient expectations, and financial constraints. Careful treatment planning avoided lengthy and inefficient procedures.

The clinical success of porcelain laminate veneers (PLVs) depends on many clinical and technical factors, from planning to execution, among which adhesive cementation is of significant importance. This procedure carries many risk factors if not optimally executed. The objective of this study was to document the clinical parameters affecting successful cementation procedures with a focus on the adhesive strength, integrity, and esthetics of the PLVs. A literature search was conducted through MEDLINE, complemented by a hand search using predefined keywords. Articles published in English between 1995 and 2023 were selected. According to this review, the success and longevity of PLVs rely in great part on the implementation of a precise cementation technique, starting from field isolation, adequate materials selection for adhesion, proper manipulation of the materials, the seating of the veneers, polymerization, and elimination of the excess cement. Several clinical steps performed before cementation, including treatment planning, preparation, impression, and adequate choice of the restorative material, could affect the quality of cementation. Scientific evidence suggests careful implementation of this process to achieve predictable outcomes with PLVs. The short- and long-term clinical success of adhesively luted PLVs is tributary to a deep understanding of the materials used and the implementation of clinical protocols. It is also contingent upon all the previous steps from case selection, treatment planning, and execution until and after the cementation.

This in vitro study aimed to evaluate the final shade of translucent zirconia laminate veneers with varying thicknesses over teeth with different shades. Seventy-five chairside computer-aided design/computer-aided manufacturing (CAD/CAM) shade A1 third-generation zirconia dental veneers, with thicknesses of 0.50 mm, 0.75 mm, and 1.00 mm, were placed on resin composite teeth with shades ranging from A1 to A4. The laminate veneers were divided into groups based on thickness and background shade. All restorations were evaluated with a color imaging spectrophotometer, to map the veneer surface from A1 to D4. Regardless of the thickness or background shade, all dental veneers showed color alteration from the original shade. Veneers with 0.5 mm thickness tended to display the B1 shade, while veneers with 0.75 mm and 1.0 mm thickness primarily exhibited the B2 shade. The thickness of the laminate veneer and background shade significantly modified the original shade of the zirconia veneer. One-way analysis of variance was performed and a Kruskal-Wallis test was used to determine the significance between the three veneer thicknesses groups. The results indicated that the thinner restorations showed higher values with the color imaging spectrophotometer, suggesting that thinner veneers may result in more consistent color matching. This study underscores the importance of carefully considering thickness and background shade when selecting zirconia laminate veneers, to ensure optimal color matching and overall aesthetic outcomes.

PURPOSE: Most veneers are mixed targeted restorative space (MTRS)-type restorations that are partially within the original tooth and require inconsistent preparation depths. This study aimed to evaluate the accuracy of the preparation depth for MTRS veneer preparation. METHODS: MTRS veneer preparation models were developed using the twisted maxillary central incisor (MCI) as the original tooth and the standard MCI as the waxing. Veneer preparations were performed using freehand (MF), silicone (MS), thermoplastic (MT), 3D-printed uniform (MD), and auto-stop (MA) guides. The prepared and original MCI were scanned and superimposed using a custom-made base. The mean absolute differences (MADs) were measured to evaluate the accuracy of the preparation depth. Statistical analysis was performed using the multivariate analysis of variance (MANOVA) test (α=0.05). RESULTS: The accuracy of the preparation depth was 0.237±0.090, 0.191±0.099, 0.149±0.078, 0.093±0.050, and 0.059±0.040 mm in MF, MS, MT, MD, and MA, respectively. The MADs between the groups were significant (P<0.05). The accuracy of the trial restoration was 0.140±0.081 mm in the MS, and the accuracy of the guiding tube was 0.055±0.033, 0.036±0.011, and 0.033±0.010 mm in the MT, MD, and MA, respectively. CONCLUSIONS: In MTRS veneer preparation for MCI, tooth preparation guides improved the accuracy of the preparation depth by visualizing the TRS profile and providing clear measurement points. The accuracy of the guide is influenced by its flexibility, and the accuracy of the preparation depth is affected by the accuracy of the measurement points.

Nowadays, the ceramic veneer approach can be considered more predictable than direct composite veneer. To date, there is a lack of studies comparing the clinical performance of anterior veneers cemented on vital teeth (VT) and non-vital teeth (NVT). This longitudinal clinical study investigated the performance of ceramic veneers in VT or anterior NVT. A total of 55 patients were evaluated in the study. Two groups were defined based on the vitality status of the teeth (93 teeth-vital and 61 teeth-non-vital). The United States Public Health Service (USPHS) criteria were used to assess the clinical status. The data were evaluated statistically with the Mann-Whitney U test. All restorations were considered acceptable, and only one veneer in VT failed for the criteria of secondary caries. There were no statistically significant differences in any of the criteria evaluated (p ≤ 0.671). The ceramic veneers evaluated showed a satisfactory clinical performance both in VT and NVT.

Objective: The purpose of this study was to evaluate the cervical marginal fit of porcelain laminate veneer (PLV) restorations made from two different types of CAD/CAM ceramic laminates: CEREC C PC and E.max (LD). Materials and Methods: This in-vitro experiment used a total of 32 human maxillary first premolars that were clean and free of any cracks or caries, extracted for orthodontic purposes. The samples were divided in a random way into two study groups: A and B (n = 16). Each sample was mounted on a dental surveyor and a silicon impression was made to create a silicone index for each tooth in both groups. Standardized preparation was carried out for all the samples by using preparation bur kit for the ceramic veneer system. Subsequently, digital impressions were made for all the samples by using Trios 3 shape intraoral camera (Sirona Dental Systems). The design of veneer restorations was made using Sirona inLab CAD SW 16.1 with CEREC inLab MC XL (Dentsply, Sirona Dental Systems, Bensheim, Germany). The veneer restorations were cemented using 3M RelyX veneer resin cement (3M ESPE, Seefeld, Germany) and the samples kept in distilled water for two weeks at 37 °C. All the specimens were subjected to thermocycling in a water bath with temperature varying between 5 °C and 55 °C for 500 cycles. The cervical marginal fit of veneers was evaluated by a digital microscope after sectioning the embedded teeth in acrylic resin. Results: The lowest mean of cervical marginal gap was recorded for Group A (91.59431 ± 1.626069) which was restored with CEREC CAD/CAM veneers, while the highest mean value of the gap was recorded for Group B (106.48863 ± 2.506684) which was restored with IPS E.max CAD. The t-test showed that the type of porcelain veneer restoration had a highly significant effect on the cervical marginal fit (p ≤ 0.01). Conclusions: CEREC CAD/CAM veneers showed smaller cervical marginal gaps, indicating a better fit compared to the IPS E.max CAD.

PURPOSE: The use of porcelain laminate veneers (PLVs) is a minimally invasive technique that is often used in restorative dentistry due to esthetic considerations. The aim of this study was to evaluate PLVs according to the Fédération Dentaire Internationale (FDI) World Dental Federation clinical criteria. MATERIALS AND METHODS: This clinical study included 11 patients (7 female and 4 male) who had been admitted to the Usak University Faculty of Dentistry between February 2019 and February 2021. Before taking part in the study, patients were informed about alternative treatment procedure options. A total of 30 PLVs were fabricated and cemented, and patients were evaluated according to the FDI criteria (with a follow-up after 2 years). RESULTS: Restorations were evaluated by two trained researchers. During the follow-up evaluation, no fractures or cracks were observed in any restoration. Further, 73% (n = 22) of the PLVs had perfect marginal adaptation and only 27% (n = 8) had small marginal fractures that could be removed by polishing. Moreover, 57% (n = 17) of the PLVs were evaluated as a good color match (no difference in shade and/or translucency), whereas 33% (n = 13) had only minor deviations. In periodontal examinations, 23% (n = 7) of the PLVs had no plaque, inflammation, or pockets, whereas 77% (n = 23) had minor plaque inflammation and no pocket development. CONCLUSIONS: As there were no fractures, fails, or need for removal of the restorations after 2 years, PLVs showed clinically satisfactory performance. The performance of PLVs can be considered highly favorable due to the minimally invasive and periodontally-compatible nature, with a good color match when carefully planned.

Purpose This retrospective study aimed to evaluate the survival and success rates of ceramic partial laminate veneers. Scanning electron microscopy was used to evaluate fractures and marginal defects.Methods In total, 31 patients received 79 partial laminate veneers on the maxillary anterior teeth. After adhesive luting, restorations were evaluated by calibrated clinicians for up to eight years using modified United States Public Health Service (USPHS) criteria. In addition, epoxy resin replicas were fabricated from silicone impressions and analyzed using scanning electron microscopy. Survival analyses were performed using the Kaplan-Meier and log-rank tests (α = 0.05). Success was analyzed in percentages by comparing the baseline and last follow-up.Results The cumulative survival rates were 100% after 1 year; 95.9% (SE 2.8%) after 5 years; and 61.4% (SE 25.3%) after 8 years. No significant differences (P > 0.05) were observed between functional and non-functional restorations. Changes in the USPHS criteria evaluation were only observed for adaptation: 12.5% (SE 4.7%), marginal discoloration: 4.2% (SE 3.0%), color match: 4.2% (SE 3.0%), and fractures: 16.7% (SE 5.3%). Scanning electron microscopy evaluations revealed undetected initial cracks and deficiencies in the restorations.Conclusions Partial laminate veneers displayed good survival rates during the long-term follow-up. The main problems observed were related to the quality of the margins, color mismatch, and restoration integrity. However, in most cases, restoration replacement was not required.

The value of having excellent teeth has increased in today's culture, even from a merely cosmetic perspective. There are several ways to address issues that arise in the area of high aesthetic sensitivity. Every modality has its benefits and drawbacks. One of the most aesthetically pleasing restorations is porcelain laminate veneers (PLVs). It has been demonstrated that the use of PLVs to address functional and/or aesthetic issues is a viable treatment option, particularly in the anterior aesthetic zone. As long as suitable procedures and guidelines are followed, these restorations are conservative and long-lasting. A patient with an anterior diastema is the main focus of this case study. This case report is essential for expanding dental knowledge, training dental practitioners, proving the effectiveness of treatments, and directing treatment choices for instances that are comparable to it. PLVs were used on the patient to close the diastema in the anterior maxillary arch and achieve a good cosmetic outcome.

Background Porcelain is considered the most esthetic and biocompatible material in dentistry with the ability to imitate sound enamel. Research into porcelain laminate veneers has focused mainly on technical or aesthetic aspects, rather than the reaction of the surrounding soft tissues. This study aims to evaluate the knowledge, attitudes, and practice regarding patient maintenance of soft-tissue alternation occurring after the placement of veneers. Methodology A descriptive, cross-sectional survey was chosen for this study to identify patient gingival alternation that occurred after the placement of veneers. Statistical analysis was performed using R version 3.6.3. Counts and percentages were used to summarize the study variables. The chi-square test was used to assess the association between categorical variables. Adverse events included hyperplasia, gum recession, change in color, change in taste/smell, toothache, and redness. Spearman's correlation was used to assess the association between hygiene, awareness, and the number of adverse events experienced after installing the veneers. Hypothesis testing was performed at a 5% level of significance. Results The study questionnaire was completed by 98 respondents. Males and females represented 34.7% and 65.3% of the study sample, respectively. One-half (53.1%) of the respondents were 30-40 years old, and one-quarter were 41-50 years old. Only one-half of the respondents were satisfied with the final result after installing the veneers (53.1%). Satisfaction was not significantly different between males and females (p = 0.818). Results showed better awareness and dental hygiene in females. Only 11.8% of the males reported visiting the dentist due to gingival problems before installing the veneers compared to 35.9% of the females (p = 0.021). The most common problems reported by the respondents were hyperplasia (62.2%) and a change in smell/taste (66.3%). Other common problems included color change (58.2%), toothache (59.2%), and redness (55.1%). Satisfaction was significantly higher in patients who did not experience gingival hyperplasia (70.3%) than in those who did (42.6%). Experiencing toothache and redness in the gums surrounding the lenses were associated with lower satisfaction (p < 0.001). The use of antibiotics was not associated with satisfaction (p = 0.495). Conclusions Our study indicated a low level of awareness and satisfaction with porcelain laminate veneer placement. There was a statistically significant association between dental hygiene and awareness scores. Dental hygiene was also positively associated with satisfaction with dental veneers. A lower awareness score was associated with lower satisfaction. Further, higher awareness was associated with higher satisfaction.

BACKGROUND: ceramic veneers, crowns, and other types of restorations are often made using either the press heating technique or the subtractive method. The advent of lithography-based ceramic manufacturing (LCM) allows for the manufacturing of such restorations in an additive way. METHODS: this concept paper describes the first clinical experience in the application of LCM lithium disilicate restorations in vivo for the manufacturing of classic veneers for a patient with severe tooth wear. The applied restorations were analyzed in terms of their marginal fit in metrology software (Geomagic Control X, 3D Systems). Furthermore, the feasibility of 3D printing of non-prep veneers with a 0.1 mm thickness was tested. RESULTS: the classic LCM lithium disilicate veneers were tried in the mouth cavity and demonstrated adequate esthetics and a sufficient marginal fit of 100 µm. Furthermore, the non-prep veneers with a 0.1 mm thickness could be successfully printed using LCM technology and also demonstrated an adequate fit on the model in vitro. CONCLUSIONS: the described technical approach of lithium disilicate 3D printing with LCM technology may pose a valid alternative to subtractive and analog manufacturing and be a game-changing option with the use of additive chairside ceramic fabrication.

MATERIALS AND METHODS: Forty laminate veneer restoration prepared from partial sintering zirconia of 3Y (yttria), 5Y (yttria), combined 3Y&5Y (yttria), and lithium disilicate. Specimens were assigned into four main groups according to their percentage of yttria content (n = 10) and subgrouped into two thicknesses (0.5 mm thickness and 0.3 mm thickness) (n = 5) as follows: Group I, II, III, and IV (Group I for lithium disilicate (control), Group II for 3Y zirconia, Group III for 5Y zirconia, and Group IV for combined 3Y&5Y zirconia), each of them subdivided according to their thickness into two subgroups (n = 5 for each one) and resistance to fracture for each restoration was evaluated using a universal testing machine. Data were analyzed using a one-way analysis of variance and Duncan's tests at a 5% level of significance. RESULTS: The thickness of laminate veneer restoration significantly affects the fracture resistance value of all type of laminate veneers restorations (fracture resistance mean value was highest for 0.5 mm thickness and lower for 0.3 mm thickness restorations) and yttria percentage significantly affect fracture resistance value of zirconia laminate veneer restorations (fracture resistance mean value was highest for 0.5 mm thickness of 3Y zirconia [865 N] and combined 3Y&5Y zirconia [846 N]). CONCLUSIONS: Reducing the thickness of laminate zirconia veneer restorations to 0.3 mm reduces its fracture resistance and increasing yttria percentage had an adverse effect on fracture resistance of zirconia laminate veneer restorations.

RESUMO

Background: To assess the quantity of dentin exposure detected by 3 operators with different clinical expertise for 2 designs of tooth preparation for laminate veneers: window (WI) and butt joint (BJ). Methods: 20 intact maxillary central incisors were collected and then prepared for laminate veneers to a depth of 0.6 mm, with a cervical mini-chamfer finish line of 0.3 mm. Each prepared tooth was analyzed by 3 operators with different expertise: undergraduate student (ST), general practitioner (GP), and prosthodontist (PR), at sight under magnification. Besides descriptive statistics (CI 95%), 2-way ANOVA and Games−Howell tests were used to analyze differences among groups (α = 0.05). Results: The means of percentage and area of detected dentin exposure were WI = 30.48%, 21.57 mm2; BJ = 30.99%, 21.97 mm2; ST/WI = 22.82%, 16.44 mm2; GP/WI = 58.05%, 40.64 mm2; PR/WI = 10.55%, 7.63 mm2; ST/BJ = 28.99%, 20.83 mm2; GP/BJ = 40.56%, 28.32 mm2; PR/BJ = 23.42%, 16.75 mm2. Significant differences were found between ST/WI vs. GP/WI (p = 0.005) and GP/WI vs. PR/WI (p < 0.001). Conclusions: There was no difference in detection of exposed dentin among operators with different expertise for BJ preparation, whereas differences were found between the general practitioner and the other 2 operators in WI. Moreover, the quantity of exposed dentin was not related to different tooth preparation designs.

OBJECTIVE: The aim of this study was to evaluate the effect of different polishing systems on the colour stability of different laminate veneer blocks for chair-side CAD-CAM after ultraviolet (UV) ageing. METHODS: About 240 sample (13 × 13 × 0.7) were prepared from four different CAD-CAM blocks IPS e.max CAD (IP), IPS Empress CAD (IE), Mark II (M) and Lava Ultimate (LU)) with A1 shade. Each group was divided into five subgroups (n = 12) according to the control (C) and four different polishing system: Sof-Lex (S), Edenta (E), Identoflex (I) and Zircon Brite (Z). Surface roughnesses (Ra) were measured by a profilometer before and after polishing. The baseline colour values were recorded according to the CIELab system. The colour coordinates (L*, a* and b*) of the samples were measured before and after UV ageing, and colour differences (ΔE00) were calculated by using the CIEDE2000 colour difference formula. A two-way ANOVA and Tukey test methods were used to analyse the data (α = 0.05). RESULTS: A significant interaction was observed between CAD-CAM blocks and polishing system (p < 0.001). The higher Ra and colour difference was calculated in LU in the Z group after polished and the lower Ra and colour difference was calculated in the IP in the S group after polished. For the IP group, the glazing procedure showed higher Ra and ΔE values with Z group than polished S system, whereas there is no statistical differences. CONCLUSION: It is concluded that polishing with the Sof-Lex system improves significantly smoothness and colour stability. However, there is no success in using the Zircon Brite polishing system.
